Position Overview

As a Registered Nurse Case Manager in the Enhanced Care Management (ECM) program, you'll play a crucial role in a multidisciplinary team focused on delivering whole-person care coordination for high-risk Medi-Cal members. You'll conduct health and behavioral assessments, develop individualized care plans, and collaborate closely with providers, behavioral health clinicians, community health workers, and care navigators. Your efforts will help improve patient outcomes and link individuals to essential services and community resources.

Position Highlights

✓ Salary: $95,000–$105,000, depending on experience

✓ Full-time, Monday–Friday schedule (8:00 AM–5:00 PM) — no nights or weekends

✓ Sign-on Bonus Available

Compensation & Benefits

    The ideal candidate is a compassionate RN with a degree in Public Health Nursing and a BSN, who thrives in a collaborative and culturally sensitive environment.
