Position Overview
As a Case Manager, you will play a vital role in supporting women throughout their treatment journey. You will help clients access essential services, coordinate referrals, and develop critical life skills for long-term success. Your day-to-day tasks will include conducting assessments, monitoring treatment progress, providing advocacy, and collaborating with a multidisciplinary team to deliver coordinated care. This role requires a blend of compassion, organization, and advocacy as you work closely with residents, counselors, and community providers.

Ideal Candidate
The ideal candidate is a compassionate and organized professional who thrives in a supportive team environment. You should possess a Bachelor’s degree and hold or be eligible for a credential such as LSW, LCDC-II, or CDCA. A Licensed Social Worker is strongly preferred. Previous experience in mental health, addiction treatment, or residential settings is beneficial. Strong communication, advocacy, and community resource navigation skills are essential, along with a valid driver’s license and insurability.
